Lake Health Madison Campus

The Madison Campus* is home to a 24/7 Emergency Department, the Lake Health Physician Group Family Practice – Madison, the Lake Health Physician Group Pediatrics, and walk-in and occupational health services provided by Dr. Hayek. In addition, the campus offers a variety of services for your health care needs.

Radiology

•    Routine X-rays
•    CT Scans   
•    Ultrasound (all adult procedures); same-day and stat procedures receive same-day results

Physical Therapy

•    A full range of evaluation and treatment options for all diagnoses (physician order required)
•    Post-operative rehabilitation can be continued on an outpatient basis with physician referral and insurance approval
•    Services include traction, ultrasound, exercise programs to decrease pain and improve mobility, and more

Clinical

•    Provides lab services for outpatients and occupational health patients
•    Exceptional phlebotomists (technicians who draw blood)
•    No appointment necessary for most tests
•    All tests require a physician order

Walk-in Care

Dr. Hayek provides walk-in care and occupational health services.

•    DOT drug testing/collection
•    Breath Alcohol testing
•    DOT certified urine collectors
•    Certified Breath Alcohol Technicians
•    Digital Fingerprinting for BCI of Ohio & FBI

Fast Track Clinic

Have you had a sports related injury in the past 2 - 4 weeks? Lake Health's Fast Track Orthopedic and Sports Injury Clinic is held on Monday and Wednesday mornings from 7:30 - 10 am at the Tyler Blvd. Urgent Care Center and on Fridays from 8 - 10:30 am and the Madison Campus Physician Pavilion.

An athletic trainer will perform an evaluation and administer treatment in accordance with state laws and physician guidance.

The athletic trainer discusses each patient with a physician. Through this consultation, treatment decisions are made and followed. These treatment decisions can include, but are not exclusive to, further evaluation by the physician, x-rays, rehabilitation, application of bracing, and scheduling of further testing.
