Benefit Overview

Medical Plan

Team members may elect single or family medical coverage. Team members scheduled to work 40 or more hours per pay are eligible for single or family plan coverage beginning the first of the month following date of hire.

Dental Plan

Team members scheduled to work 40 or more hours per pay are eligible to elect single or family plan dental coverage beginning the first of the month following date of hire.

Vision Plan

Team members scheduled to work 40 or more hours per pay are eligible for single or family plan vision coverage beginning the first of the month following date of hire.

403b Retirement Savings Plan

Lake Health team members are eligible to participate in this plan upon hire. To be eligible for the employer matching contributions a team member must have completed one year of service and worked a minimum of 1000 hours. Team members enter the plan the following January 1 or July 1.

Basic Life Insurance and AD&D

Lake Health team members are insured for 1x their annual salary in term life insurance plus up to an additional 1x their annual salary for an accidental death or dismemberment. Team members scheduled to work 40 or more hours per pay are eligible for this benefit on the first day of the month following six months of service.

Optional Life Insurance

Lake Health team members may purchase additional life insurance. This is not guaranteed issue, however, and proof of insurability may be required to purchase this benefit. Team members scheduled to work 40 hours or more per pay period are eligible for this optional insurance plan coverage beginning the first of the month following date of hire.

Optional Spouse & Dependent Life Insurance

Team members may purchase optional term life insurance for a spouse and/or dependent child(ren). Team members scheduled to work 40 or more hours per pay are eligible for plan coverage beginning the first of the month following date of hire.

Short Term Disability

Lake Health provides Short Term Disability at no cost to eligible team members. This plan will cover 50% of a team member’s annualized base wage at time of disability up to 150 days after a 30-day elimination period, provided the team member has completely exhausted their PTO bank. Team members scheduled to work 64 hours or more per pay period are eligible for this coverage beginning the first day of the month following one year of eligible employment.

Long Term Disability

Long Term Disability is provided at no cost to eligible team members and will cover 50% of the team member’s gross annual salary up to $10,000 per month after a six month elimination period in the event of a long-term illness. Team members scheduled to work 64 or more hours per pay are eligible for this benefit on the first day of the month following one year of service.

Health Care Flexible Spending Account

The Health Care Flexible Spending Account allows team members to use pre-tax dollars to pay for out of pocket medical, dental and vision expenses, which are not otherwise covered by your employer-provided insurance coverage’s.

Dependent Care Spending Account

Dependent Care Flexible Spending Accounts enable team members to pay with pre-tax dollars the expenses incurred for the care of a child, elderly parent or other dependents.

Tuition Reimbursement

Full time and part-time team members may be reimbursed for qualifying undergraduate, graduate and advanced certification educational expenses during a calendar year. Team members must apply and be approved for tuition reimbursement prior to beginning course work or program. Team members scheduled to work 40 hours or more per pay are eligible for this benefit on the first day of the month following date of hire, however reimbursement will not occur until (6) months of employment have been completed.

Paid Time Off (PTO)

Team members accrue PTO on a bi-weekly basis and may begin using it after three months of service. PTO accrual rates vary by position. PTO encompasses vacation, sick and holiday time. Team members scheduled to work a minimum of 40 hours per pay are eligible for this benefit.

Super Savers Program

Various discounts are available to LHS team members from area businesses. Discounts are also offered for wellness programs, health and education programs, cinemas, amusement parks and more. Check with the Human Resources Dept. for more information.

Voluntary Benefits

The option to purchase voluntary Universal Life, Cancer, Accidental Death, Critical illness and Supplemental Short-Term Disability is available to interested benefits eligible team members. Please check with Human Resources for more information regarding these plans.
