The Lake Health Center for Comprehensive Rehabilitation (CCR) is an inpatient facility providing rehab nursing and physical, occupational and speech therapy to patients with a wide variety of physical disabilities. Our goal is your safe return to independent living.
Acute care for adults in a healing space
We provide extensive rehabilitation for patients disabled by serious illness, injury or surgery. Located in the Continuing Care Campus* in Concord Township, the CCR features a soothing environment designed to promote healing. The unit includes 12 spacious private patient rooms, a gym for therapy, and a separate “activities of daily living” space to monitor and strengthen your day-to-day functioning.
An intensive program to help you regain independence
Before admission, you’ll be evaluated to determine your ability to tolerate and benefit from the CCR’s intensive rehab program. You must be able to tolerate at least three hours of therapy a day for a minimum of five days a week.
Each day, you’ll work with a variety of therapists to regain function and skills, from grooming and bathing, to getting in and out or chairs, to improving cognitive skills. Patients typically stay in the CCR about two weeks depending on their needs, potential and goals. Your team will meet weekly to monitor your progress.
We also recognize the important role your family plays in your recovery. We involve family members throughout the rehabilitation process and train them to meet your special needs at home.
Individualized care from a specially trained team
After your initial evaluation, we’ll create your team of physicians, physical therapists, occupational therapists, speech and language pathologists, social workers or case managers, rehabilitation nurses, or other specialized services as needed. Working closely with you, they’ll develop a treatment plan to meet your needs and rehabilitation goals.
