Heart Failure Clinic
We’ll help you live a better life
Heart failure is a serious condition, but many people with heart failure lead full, enjoyable lives when their condition is managed with medications and a healthy lifestyle.
The Lake Health Heart Failure Clinic is here to help you monitor and assess your condition so you can live an active life. Located at West Medical Center and TriPoint Medical Center, our clinic staff can help you better understand and manage your condition.
Working with your doctor, our staff will develop a personalized treatment plan that’s right for you and your symptoms. At each visit, our nurse practitioner will monitor and evaluate your medications, blood pressure, weight and breathing for any changes in your condition. If there is a change in your condition, our nurse practitioner will adjust your medication or treatment plan.
Ongoing support and education
During your visit, our nurse practitioner can help you understand your disease by explaining the symptoms and physical effects of your condition, offer helpful nutrition counseling, and answer any questions. The clinic staff also provides access to a valuable support network, including dietitians, social workers, and physical and respiratory therapists. The staff can even coordinate your physician and therapy visits, too.
