Lake Health’s patient portals give you easy access to test results, upcoming appointments, recent visit records and more. Use the Lake Health Connect: My Health portal to see information about your care at Lake Health urgent cares; TriPoint, Lake West or Madison Emergency Departments;  overnight stays at TriPoint or Lake West medical centers; and radiology or lab tests taken at any Lake Health facility other than your doctor’s office. 

Use the Physician Office portal for information about visits to your Lake Health Physician Group doctor’s office, in-office labs or x-rays, or to secure message the office.

Refilling your prescription is simple with our Quick Refill option for Lake Health’s retail pharmacies at TriPoint and Lake West medical centers.

Most major insurance providers include Lake Health in their coverage, however there are more narrow networks where access maybe restricted. Please check with your insurance carrier to make sure Lake Health is in network.
