Diabetes Care and Education

We’re committed to helping you manage your diabetes with expert treatment and education. The certified diabetes educators at Lake Health can help you control your diabetes with a wide range of support services, available to anyone 18 years and older with type 1 or type 2 diabetes. A physician’s referral is required.

Our self-management education offered at no cost. If you have coverage questions about other diabetes services, call your insurance company or call our diabetes education team at 440-354-1622.

  • PreventT2 –Diabetes Prevention Program
  • Medical Nutrition Therapy services for people with diabetes who have chronic kidney disease, hypertension, hyperlipidemia, weight-loss needs or other conditions impacted by nutrition.

We also provide Medical Nutrition Therapy services for people with diabetes and chronic kidney disease, available through a physician referral. Chronic kidney disease is progressive and serious, but with proper care it can be managed and its progression slowed.

PreventT2 Diabetes Prevention Program

If you have prediabetes or other risk factors for type 2 diabetes, now is the time to take charge of your health and make a change. The PreventT2 lifestyle change program can help!

PreventT2 is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). It features an approach that is proven to prevent or delay type 2 diabetes. By improving food choices and increasing physical activity, you can lose 5 to 7 percent of your body weight — that is 10 to 14 pounds for a person weighing 200 pounds. If you have prediabetes, these lifestyle changes can cut your risk of developing type 2 diabetes in half.

How the PreventT2 Program Works

As part of a PreventT2 group, you will work with a trained lifestyle coach and other participants to learn the skills you need to make lasting changes. You will learn to eat healthy, add physical activity to your life, manage stress, stay motivated, and solve problems that can get in the way of healthy changes. PreventT2 is a year-long program with weekly meetings for the first 6 months, then meetings once or twice a month for the second 6 months to help you maintain your healthy lifestyle changes. By meeting with others who have prediabetes you can celebrate each other’s successes and work together to overcome obstacles.
