Diabetes Care and Education
We’re committed to helping you manage your diabetes with expert treatment and education. The certified diabetes educators at Lake Health can help you control your diabetes with a wide range of support services, available to anyone 18 years and older with type 1 or type 2 diabetes. A physician’s referral is required.

- Basics of Diabetes – Self-management Education Program
- Matching Insulin to Carbohydrates Program
- Blood Glucose Monitoring Program
- Individual and small group classes
- Inpatient diabetes care
- Cooking demonstrations
- Community presentations for groups and organizations
- Prevent T2 –Diabetes Prevention Program
- Medical Nutrition Therapy services for people with diabetes who have chronic kidney disease, hypertension, hyperlipidemia, weight-loss needs or other conditions impacted by nutrition.
We also provide Medical Nutrition Therapy services for people with diabetes and chronic kidney disease, available through a physician referral. Chronic kidney disease is progressive and serious, but with proper care it can be managed and its progression slowed.
