The certified diabetes educators at Lake Health can help you control your diabetes with a wide range of support services, available to anyone 18 years and older with type 1 or type 2 diabetes. A physician’s referral is required.

Medical Nutrition Therapy services for people with diabetes who have chronic kidney disease, hypertension, hyperlipidemia, weight-loss needs or other conditions impacted by nutrition.
We also provide Medical Nutrition Therapy services for people with diabetes and chronic kidney disease, available through a physician referral. Chronic kidney disease is progressive and serious, but with proper care it can be managed and its progression slowed.
