Cardiac Rehabilitation
Lake Health’s Cardiac Rehabilitation is a comprehensive and progressive program to help you recover from a cardiac event (such as a heart attack) or procedure and make changes to lead a healthy life moving forward.
Our expert cardiac rehab team empowers you to make changes to live a healthier life and reduce the risk of another heart event.
You qualify for outpatient cardiac rehabilitation if you were hospitalized recently with a heart attack, congestive heart failure, angioplasty and/or stent, heart catheterization with angina or open-heart surgery, including bypass, valve surgery or heart transplant. Cardiac rehab is a physician referred program covered by most insurance plans. Our program is certified by the American Association of Cardiovascular and Pulmonary Rehabilitation.

About Cardiac Rehab
Phase One of cardiac rehab begins in the hospital during your recovery from a heart event or surgery. During this time, our cardiac rehab team visits you to share information, support you and guide you in doing low-level daily exercise to help lead you on the road to recovery with confidence.
We’ll educate you and your family about your heart condition, how lifestyle and risk factors have impacted your health and how to change your lifestyle to reduce future risk. We can also provide one-on-one education and help connect you to care at home after a surgery.
Phase Two of cardiac rehab is a structured exercise and lifestyle change program lead by a team of registered nurses, exercise physiologists, a registered dietitian and a respiratory therapist. Our outpatient cardiac rehab program is located inside the University Hospitals Brunner Sanden Deitrick Wellness Center in Mentor.
Phase Two is much more than guided fitness. It features:
- Nutrition evaluations and counseling to help you identify heart healthy food choices and help you lose weight, if needed.
- Connecting you with programs such as smoking cessation and stress management classes to help reduce your risk factors for poor heart health.
- A comprehensive education program to help you develop healthy habits.
- A positive support system for you and your loved ones, with encouraging peer support.
After you complete Phase Two, you have the option to stay connected and continue on your road to healthy living by continuing as a LiveHealthy fitness center member.
