Going to a doctor’s office appointment can be difficult for people who are elderly or living with a debilitating condition. The Lake Health House Calls program offers personalized, in-home primary care and palliative care to patients managing chronic illness or recovering from a recent hospitalization. Care is available to patients in the comfort of their own home, assisted living or nursing home facility, depending on their needs.
Nurse Practitioners
On-call 24 hours a day, nurse practitioners can manage your health care as well as collaborate with other disciplines to provide effective and seamless care.
With a focus on illness prevention and maintaining quality of life, nurse practitioners provide supportive services to manage symptoms, order and interpret diagnostic tests and therapies, prescribe and educate about proper medication use, and facilitate communication among all members of the health care team, including the patient and family. House call visits are ongoing for as long as the patient and medical provider feel they are necessary or helpful for the individual.
