Comprehensive Rehabilitation
The Lake Health Center for Comprehensive Rehabilitation (CCR) is an acute inpatient rehabilitation facility providing rehab nursing and physical, occupational and speech therapy to patients who can benefit from intensive therapy to safely return to independent living.
The CCR is designed to promote healing, with 12 spacious, private patient rooms, a gym for therapy, and a separate “activities of daily living” space to monitor and strengthen your day-to-day functioning.
Working closely with you, our multi-disciplinary team develops a treatment plan to meet your needs and rehabilitation goals. Each day, you’ll work with a variety of therapists to regain function and skills, from grooming and bathing, to getting in and out of chairs, to improving cognitive skills. Patients typically stay for about two weeks.
We also involve family members throughout your rehabilitation and train them to meet your needs at home.

Your Team
Your team of physicians, rehab nurses, physical therapists, occupational therapists, speech and language pathologists, social workers or case managers, rehabilitation nurses and other experts will work to help you regain coordination, balance, strength and function. We’ll help you manage your condition or adjust to changes resulting from illness, injury or surgery such as:
- Stroke
- Amputation
- Hip fracture
- Complex orthopedic surgeries
- Neurological conditions
- Spinal cord injury
- Multiple traumas
- Brain injury
- Bilateral joint replacement (Patients must have other ongoing medical issues that require continued hospitalization and close physician supervision)
